Nebraska Chronic Disease Summit

The Nebraska Department of Health and Human Services Chronic Disease Prevention and Control Program is pleased to announce that we will be hosting a Nebraska Chronic Disease Summit. The Summit will focus on efforts occurring at the federal, state, and local level to reduce risk factors associated with childhood obesity and adult obesity, diabetes, heart disease, and stroke.

Sessions will focus on creating healthy environments in workplaces, schools, and the community and connecting health systems with community organizations to reduce complications from multiple chronic conditions. The Summit will also feature interactive skills building sessions offering participants tools, resources, and increased confidence to actively implement the skills learned into their daily work.

The summit will feature national, state and local speakers highlighting current efforts toward preventing and controlling heart disease, diabetes and obesity. Workshops will also feature skills building opportunities to support the implementation of public health initiatives.

Confirmed national speakers include:

• Mark Fenton – a public health, planning and transportation consultant who works with organizations and communities to build environments, policies and programs where more people walk and bike

• Tim McNeill – an independent healthcare consultant in program development and sustainability who has worked with the Centers for Medicare and Medicaid Services and the U.S. Administration on Aging

• Jill Feldstein – the director of Penn Center for Community Health Workers, a center dedicated to improving health outcomes among low-income individuals by providing personal, community-based support

• Brian Isetts – a professor studying the outcomes of medication therapy management services provided in team-based care settings and focuses on the exchange of electronic health information to redesign the nation’s medication use system

• Melissa Fahrenbruch – a UNL graduate who serves as the lead health education specialist for the Centers for Disease Control and Prevention

Location: Embassy Suites Omaha/LaVista Hotel and Conference Center

Cost: $135 – Includes breakfast and lunch both days and afternoon snack June 6th

Lodging: A block of rooms has been reserved at the Courtyard Marriott at the group rate of $91 (plus tax) per night. Reservations may be made by calling 402-339-4900. This block rate is available on a first come, first serve basis until May 19, 2017. After this date, reservations will be accepted on a space and rate availability basis.
